If you would like to fill out the form on your computer and email it to the school, use this form:

If you would like to print the form, fill it out, and then return it to the school's attendance office, use this form:

Complete all the information on the form and return it to the attendance office or email it to our attendance clerk, Brandon Walker, at [email protected]. We do not take attendance information over the phone.

Please make sure that all documents contain the Student ID Number. This is the only way an absence can be excused.
