The state of Florida requires that all students entering 7th grade must have the TDAP (Tetanus-Diptheria-Pertussis) Vaccine. Your child must have this vaccine in order to start 7th grade at Maitland Middle School.

If your child has had the TDAP Vaccine, please get the Form 680 from your doctor and email to [email protected], fax to (407) 623-1474, or drop it off to us at the front office.
If your child has not yet had the TDAP Vaccine, OCPS has partnered with Health Hero Florida to offer a TDAP Vaccine Clinic at Maitland Middle School on Friday, April 30, 2021.

Face to Face StudentsForms went home with Face to Face students on Friday, April 9. Please sign and return the 2021 TDAP Consent Form to us if you would like this free immunization for your child right here at the school to fulfill this state requirement. If you need another copy of the form, it is available from the link below.

LaunchED Students: LaunchEd students are also eligible to participate in this vaccine clinic and can print the form from the link below. We have a date for the clinic, but not an exact time. Nurse Kelly will coordinate a time with you as the date approaches.
