Welcome to the Registrar’s Page for Maitland Middle School. On this page, you will find everything you need to guide you through the enrollment (registration), withdrawal, or transfer process for your student. This page also contains information about the required Tdap vaccination for 7th grade students and the required verification of residence for 8th grade students.

    The answers to the most common questions are contained in the information below. For those without computer or internet access, please gather all required documents and then contact Nan Bressler at the number listed below.

    2025-2026 Students from Other Schools

    Students registering from other schools (such as out-of-state, out of district, or private) will not be able to enroll until the 2024-2025 year is completed at their current school, the student is withdrawn, their final grades are available showing promotion, and transfer documentation is provided.

    How to Complete Registration Forms

    1. Download blank registration packet and save to your computer. Do not fill out in Chrome, use Adobe Reader only. Data filled out in browser will not save when attached to an email.
    2. Open saved document.
    3. Fill in blanks.
    4. Save completed registration packet to attach to email.

    Student cannot be registered until all documentation is received. Therefore, please hold off on sending individual documents. When all required documentation is complete, please attach the following items to an email and send to Nan.Bressler@ocps.net.

  • Withdrawal Process

    THE WITHDRAWAL PROCESS IS DONE VIA EMAIL

    Please Note that Final 2024-2025 Report Cards will not be available until July 11th

    STEP 1

    If you need to withdraw your student, please email the following information to Nan.Bressler@ocps.net

    • Student’s name
    • Student’s date of birth
    • Name of new school
    • Student’s last day at Maitland Middle
    • Photo of your ID/Driver’s license for identification purposes

    STEP 2

    Please return the laptop and charger to the front office on the last day of attendance. This must be done in order for the student to be withdrawn.

  • 7th Grade Tdap Vaccine Requirement

    OCPS Parents of Rising 7th Grade Students,

    The state of Florida requires that all students entering 7th grade must have the Tetanus-diphtheria-pertussis immunization also known as the Tdap immunization. Your child MUST have this immunization in order to start the 7th grade. For information you can visit Orange County Florida Health Government.

    Your child’s immunization will be documented on the updated form you will need to submit to your child’s school before they can attend class for the 2025-26 school year. This is also an excellent opportunity to have your child’s immunization records reviewed by a health care provider to ensure your child’s vaccines are up-to-date. 

    Please email a copy of your child’s immunization record (form DH 680) to the school registrar at Nan.Bressler@ocps.net

  • Verification of Residence

    Parents,

    In an effort to ensure all student's records are accurate and up to date, the OCPS Office of Student Enrollment is requiring that parents of rising 9th grade (current 8th grade) students provide updated current proof of residence to Maitland Middle School. The documentation must be provided to the school by close of business on April 18, 2025.

    Failure to provide updated verification of residence will result in your child's WITHDRAWAL from OCPS and you risk your student not having a schedule for the start of the 2025-2026 school year.


    To verify your address, you must provide certain required documents based on your living situation. Please click the following link to complete the Verification of Residence Google Form:

Required Documents

  • Official Birth Certificate

    • A duly attested transcript of the child’s birth record filed according to law with a public officer charged with the duty of recording births.
       

    OR

     

    Passport

    • A passport or certificate of arrival in the U.S. showing the name of the child.
       

    NOTE: Under no circumstances shall staff request a passport, visa, or any other documentation to verify the immigration status of any student.

  • Birth Certificate or Passport*
     

    • To enter kindergarten, a child must be 5 years old on or before September 1.
    • To enter first grade, a child must be 6 years old on or before September 1, and have completed kindergarten.
       

    *Other forms of age verification are permissible under Section 1003.21, Florida Statues

  • Proof of physical examination by a U.S. doctor within a year of enrollment (first day of entry at school).

     

    AND

     

    Proof of immunizations on a Form 680 (obtained from your doctor’s office or the Orange County Health Department.)

    *Students may attend school without a Florida Certificate of Immunization if they have a: 

    • Religious Exemption (Form DH 681)
    • Temporary Exemption (Form DH 680, Part B)
    • Medical Exemption (Form DH680, Part C)​​​​
  • Transcript, withdrawal form or last report card

  • Current IEP or 504 Plan

  • Enrollment takes place at the school with the school registrar if you have the following as proof of residence, at a minimum:
     

    • Own Home/Mortgage: Provide the current homestead exemption card, current property tax statement, closing contract, or warranty deed along with current utility bill, ID with current address, and current mail as your proof of residence.
       
    • Rent/Lease: Provide the current signed lease, ID with current address, current utility bill, rent payments, current mail, and renters insurance as your proof of residence.
       

    The following situations require a Verification of Residence obtained from the Office of Student Enrollment BEFORE enrolling at the school.
     

    • Living with someone who owns or rents the home:
       

    If you don’t have a lease or own your own home (share housing with someone else) you must obtain a Verification of Residence from Student Enrollment as your proof of residence by providing a minimum of the following documents:

    • Parent or Guardian's Florida driver's license or state ID showing the current correct address.
    • Another three (3) items in the Parent or Guardian's name showing the current address, such as: auto registration, current mail, bank statement, insurance, phone bill, or current pay stub.
    • Homeowner or Lessee's driver's license or state ID, homestead exemption card, property tax information, or signed lease agreement.
    • More documentation may be required depending on circumstances in a case-by-case situation.

  • Birth certificate

    If applicable, you must provide one of the following:
     

    • Court Custody Documentation (this includes divorce decrees)
    • OCPS Educational Guardianship: This documentation is provided by the Office of Student Enrollment when the parent/guardian lives outside of Orange County or adjacent counties of Brevard, Osceola, Polk, Lake, Seminole, and Volusia).

Enrollment Information

We are happy for you join our school family. Below you will find the list of required documents necessary to enroll within Orange County Public Schools. Once you have gathered all the necessary documents and completed the registration packet, you will be able to register here at the school.

Please pay special attention to the requirements for Verification of Residence and Verification of Guardianship. For some families, it may be necessary to visit the Office of Student Enrollment to receive the proper documentation BEFORE registering here at the school.
